The IMPACT project includes several participant groups. The Advisory Committee first convened on April 27, 2011. The Worcester Galaxy kick-off meeting was on June 21, 2011 and included more than 50 participants with representation across 31 organizations. From the Worcester Galaxy will be selected a subset of participants for the Phase 1 pilot project.
Learning Collaborative participants will include members of the Worcester Galaxy and phase 1 pilot participants. The primary purpose of the Learning Collaborative is to review the process surrounding care transitions and discover ways to make that process better. Understand importance of care transitions. One way this might be accomplished is to have participants walk in each other’s shoes: the sender needs to understand why the data are needed and the receiver needs to appreciate the difficulty or constraints in collecting data. Learning Collaborative participants will work to define the best practices surrounding care transitions and assist in their dissemination
